Comprehensive Program Available to
Members

(AFSA) members can now access its comprehensive group health insurance program
that offers major medical, life insurance, and optional disability and dental
coverage. This program was designed to meet the varying needs of AFSA members
and is available through Mass Marketing Insurance Companies (MMIC), which has
over 30 years of experience in the development of successful insurance programs
for professional and trade associations. Through MMIC, members will be dealing
with association health insurance specialists capable of offering the broadest
range of insurance companies, policies and plans.
MMIC contracts nationally with a number
of health insurance companies to provide a wide variety of benefits, giving it
the ability to create a customized insurance program rather than being limited
to the plans under one insurance company. Based on your needs, MMIC provides
quotes from those companies able to provide the broadest coverage at the best
possible cost.
Coverage is available to individual members and those firms with two or more employees. For AFSA members wishing to have group coverage, all active full-time employees including owners, partner and officers working at least 30 hours a week are eligible. An insured spouse and dependent children under age 19 (under age 24 if full-time student) are also eligible to apply.
Certain participation
requirements must be met and maintained. These requirements are based on the
number of eligible employees per form and are available from MMIC. The cost of
coverage for the AFSA program may be paid in whole by the employer or shared
with the employees. However, the employerÕs contribution must be at least 50
percent of the total cost.
The major medical plan
available will offer up to $5,000,000 lifetime maximum per insured with a
choice of deductibles ranging from $250 up to $5,000. Eligible hospital
expenses such as surgery and doctor services are covered. Most programs will
have a doctor office visit benefit ranging from $10 to $25 and a prescription
drug card with co-payments of $10 for generic and $20 for brand name. The plans
contain certain limitations and exclusions, which are outlined in your
